Before any cadet can participate in NJROTC physical training, field events, or competitions, they must complete the Athletic Clearance process through the official FHSAA Home Campus website.

This clearance ensures that every student is medically cleared, properly insured, and meets all district and state requirements for participation in physical activities.
Completing this process protects both cadets and the program by maintaining safety, accountability, and compliance with Hillsborough County Public Schools and FHSAA regulations.

Athletic Clearance Process – Step-by-Step Guide

 It’s very important to gather all required forms and documents before starting the online application.
Missing paperwork will delay the approval process. Make sure you have digital copies (PDF or JPEG) of each file ready to upload.

⚠️ ***Effective July 1, 2026, all first-time high school participants in FHSAA athletics must have an Electrocardiogram (ECG) screening before participation. This applies to students with no cardiac symptoms. Students with cardiac symptoms should consult their healthcare provider. An ECG completed within two (2) years prior to July 1 of the participation year satisfies this requirement. If the ECG requires further evaluation, the student must be cleared by a licensed medical practitioner trained in the diagnosis, evaluation and management of ECGs before participating in FHSAA athletic competition, practice, tryouts, or workouts. Must complete EL1 - Electrocardiogram (ECG) screening form revision 2/26. ***⚠️ 

You will need:

1️⃣ EL2 / PREPARTICIPATION PHYSICAL EVALUATION (printed, filled, scanned in PDF or JPEG format)

• Must include doctor’s stamp, signature, and date on Page 4 of the EL2 from (revision 2/26)

2️⃣ 4 Required Course Video Certificates 

• Complete the 4 free online courses at NFHSlearn

• ❗Courses Must be completed in the student’s name and dated after May 15, 2025.

1.     Concussion in Sports

2.     Sudden Cardiac Arrest

3.     Heat Illness Prevention

4.     Sportsmanship

❗Certificates will need to be uploaded later (each certificate individually (PDF or JPEG))

3️⃣ School Insurance

• Must be purchased online at: www.hcpsathleticprotection.com

• Select Basic Accident Insurance Plan – NJROTC Drill Participants ($30.00)

• Covers all NJROTC activities and summer events

• Download or screenshot your insurance ID card after purchase (it will be required to upload later)

4️⃣ Birth Certificate of the student (PDF or JPEG)

5️⃣ Government photo ID of the Parent/Guardian applying. (❗Address on ID must match the proof of residence)

6️⃣ Proof of Residence (in one document file PDF/JPEG). Examples:

• current electric/water/gas bill within the last 30 days; OR

• full, current lease (all pages); OR

• mortgage statement within the last 30 days; OR

• tax record showing homestead exemption).

7️⃣ TWO “Additional” School Specific Forms (for NJROTC) in PDF/JPEG format:

•  Enrollment & Residential History Form – Download, fill out, and sign (no e-signatures)

•  NJROTC Health Risk Screening Questionnaire - Download, fill out, and sign (no e-signatures) If you answered yes to any question on this form please continue to the second page and it must be signed by licensed physician.

1. Create Your Account

• Go to https://athleticclearance.fhsaahome.org/

• Create a new Home Campus account or log in if you already have one

• Click “Start Clearance Here”
  
  

Select:

• Year: 2026–2027

• School: Freedom (Tampa)

• Sport: JROTC Drill and Orienteering

2. Complete the Online Form

• Fill in all Student details → click Save and Continue

• Fill in all Parent/Guardian details → click Save and Continue

• Complete the Medical History section → click Save and Continue

• Fill in Program Information → click Save and Continue

• Read and complete all Electronic Signatures (❗names must match student and parent info) → click Save and Continue

3. ❗Upload All Required Documents

In the upload section, attach each of the following in the corresponding section:

• EL2 / PREPARTICIPATION PHYSICAL EVALUATION

• Birth Certificate

• Proof of Residence

• FHSAA Course Certificates (4 certificates)

• Proof of School Insurance

• Government-Issued Parent/Guardian Photo ID

4. Review and Submit

• Double-check that all files are attached and readable

• Submit your clearance application for review

• You will receive an email once your student’s clearance is approved or denied
